Job Summary: The Crisis/Addictions Support Worker will attend to crisis situations, provide support to the addictions program, complete intake, risk assessments, sassi assessments, support families with mental health warrants, mental health care of incapacity and provide support to clients who may be at risk. Help clients navigate their wellbeing by advocating, referring to other service providers. Plan and deliver programming that is suitable for the clientele in collaboration with other agencies. Working after hours for on-call as well evening programming, and community events and/or activities on a voluntary basis. Maintain a manageable case load for on-going support and provide case planning.
Job Duties:
Complete a comprehensive client assessment and service plan. Assist clients in identifying areas of need and goal development.
Complete a risk assessment and environmental screen on intake/contact.
Provide crisis intervention services through telephone or face to face contact, in the clients preferred environment.
Encourage the client in problem solving, conflict resolution, harm reduction, relapse prevention, medication management and other life skills as appropriate. Using a hands-on approach to encourage and motivate, provide life skills training as needed (i.e... cooking, cleaning, accessing, transportation etc.).
Develop comprehensive discharge plans which include relapse prevention strategies and connection to on-going internal or external supports as needed.
Provide follow-up as appropriate and provide back-up assistance to the agency programs as necessary.
Maintain up-to-date client records.
