The Wellness Intervention Officer will provide culturally sensitive, trauma-informed services to newcomer women and families during intake and initial intervention. The role involves assessing client needs, making referrals, and creating personalized wellness plans to support clients' well-being. A strong understanding of social work, community resources, and wellness strategies is required.
Responsibilities:
Conduct intake assessments with newcomer women and families to assess their wellness needs and service eligibility
Develop individualized wellness intervention plans using a trauma-informed, culturally sensitive approach
Assess clients emotional, psychological, and social well-being, identifying areas of need and support needed
Manage a client caseload, using case management to support their wellness journey.
Facilitate access to external resources and community agencies that align with the client's wellness goals
Advocate for clients to access services related to mental health, housing, employment, and other social services
Use a culturally sensitive approach to wellness support, ensuring services suit clients from diverse backgrounds
Provide interventions that incorporate trauma-informed care to address the challenges faced by newcomer families
Build and maintain partnerships with community agencies, healthcare providers, and other support services
Compile statistical data and case summaries for program evaluations and reports
Offer services during flexible hours(evenings and weekends)
Qualifications:
Bachelor in Social Work or related degree in social services, psychology, or a similar field
1-2 years of experience in settlement agencies, especially with vulnerable populations (preferably immigrants)
Proven experience in case management and client-centered, interventions, particularly within wellness
Work independently and collaborate effectively in a team
Valid Class 5 Drivers License and access to a reliable vehicle
