Description: Working under the direction and supervision of the Executive Director and/or designate, the YAP Outreach Program (Reaching Home) assists underhoused youth referred through the Coordinated Access Program. These individuals may be chronically and episodically homeless and may have a variety of challenging and often overlapping problems including, serious mental wellness issues, substance abuse, and other complex, trauma related issues. This position will work with other RYC Programs when needed and necessary in a referral format. Qualifications:
University degree, human service-related diploma, or certificate; plus two years of recent related experience, or equivalent combination of education, training, and experience and preferably 2 years case management experience. Familiarity with community resources. Current CPR/Mental Health First Aid is an asset. Must have class 5 drivers license with no probation, and current drivers abstract. Strong ability to manage stress and organize workload. Excellent communication, writing, documentation, and organizational skills. Ability to understand and maintain clientele/worker boundaries.
