Duties
Study the demographics of the community and conduct research to identify areas of health concerns in the community
Together with the nursing staff, establish care strategies for chronic diseases that are prevalent in the community
Coordinate a database of statistics and narratives for tracking purposed and efficacy of treatments and the program outcomes
Gather educational literature for clients and staff
- In the absence of the Continuing Care Aide
Provide health care services for clients/residents/families in their home environments.
Assisting clients with activities of daily living, personal care, home making services and health care.
Work with the elderly, ill or physically challenged community members
Providing a Meals on Wheels program for clients who have been assessed
Job Requirements
Experience in the Home Care field would be a definite asset
Must have or be willing to get First Aid, CPR, and AED certification.
Must have a Saskatchewan Driver License and a reliable vehicle at their disposal
Other related Duties
Reporting relevant changes in clients conditions or caregiver status to supervisor
Attending training programs and in-services to maintain/increase knowledge base
