Description

Reporting directly to the Housing Team Lead, the Support Worker is
responsible for the provisions of housing stabilization and support with intensive case management services for those that are homeless or at risk of being homeless. This position will provide supportive services to the highest acuity clients. Job Duties:
Meet with Intensive Case Managers daily to discuss client needs and schedule
Schedule and attend house viewings, provide transportation when required.
Provide transportation to appointments and programming.
Provide in home life skills mentoring (including but not limited to budget planning, meal planning, parenting, cleanliness and hygiene).
Conduct client check-ins via phone, email and in home.
Work alongside Housing Team Lead, Sohkayawin Intensive Case
Managers, and other NYFC program essential services staff to case plan and case manage clients to ensure they have access to a wide array of supports promoting a holistic approach to over-all wellbeing.
Conduct milestone SPDATs and update required consents and documentation.
Maintain and report on program statistics, complete case plan notes and database entries.
Adhere to organizational policies and procedures, as well as HIFIS and Coordinated Access policies and procedures.
