Duties include:
-Coordinate and facilitate individual and group life enrichment activities in personal homes and in the community, enhancing the quality of life for individuals living with dementia.
- Coordinate and facilitate supportive living services to clients to support health aging at home.
- Build trusting relationships with care partners to understand the needs of their loved ones and how to best support their quality of life through person-centered planning.
- Work collaboratively with other team members to ensure the needs of the clients and care partners are being met.
Qualifications:
- Rehabilitation Worker Diploma or equivalent (2 year diploma), Personal Care Aide
- Valid Class 5 Drivers Licence and a reliable vehicle that can be used for work and transporting clients
- Valid First Aid/CPR/AED certification, or obtain within three months
- TLR/PART training
- Experience working with people living with dementia and/or disabilities
- Working independently and within a team
- Problem-solving, being empathetic, patient and resilient
- Ability to work cooperatively with care partners and referral sources
- Excellent organizational and time management skills
- Strong interpersonal, networking, and relationship-building skills
- Computer literacy in Microsoft Office, Outlook, and database including file recordings and statistical reporting
