Sanctum is a program that provides transition, respite, and palliative care to HIV positive individuals.
The home support worker will provide care and support to residents with HIV, who may also be affected by mental illness, addictions, poverty, homeless or other psycho-social factors. The home support worker will embody a patient centered harm reduction care model fostering an environment of acceptance and understanding for clients.
This position is for all shifts, weekdays and weekends
Days: 07:30 - 16:00
Evenings: 15:30 - 00:00.
Nights: 23:30 - 08:00
Responsibilities
Personal care & hygiene
Assist with and support clients with medication adherence.
Establish and maintain current, accurate, confidential files
for each resident.
Follow medical instructions carefully and consistently.
Assist with meal preparation and planning.
Household duties such as cleaning and maintenance
Manage any unanticipated events or unstable situations.
Qualifications
Harm Reduction & Trauma and Violence Informed Care.
Mental health & Addictions
Cultural sensitivity
Clear communication skills, teamwork, and relationship building.
Conflict resolution and interpersonal skills
Strong Decision-making in varying situations
Degree/diploma or certificate in social work, care aid, addictions, or related field
Must retain active CPR and First Aid certification.
Must retain Food Safety Certificate
Must retain TLR Certificate
