Proven knowledge, skills and experience in providing support and advocacy to marginalized and vulnerable older adults.
Understanding of First Nation and Mtis cultures and the needs of the Mtis and First Nation Peoples within Saskatchewan will be required.
Working knowledge of the dynamics and impacts of homelessness and are competent in working with individuals from diverse cultural backgrounds.
Skills in conducting safety assessments and safety planning.
Works cooperatively and collaboratively with other community professionals, and the broader community. Experienced in organization, self-care, and general work management skills as well as an ability to work well independently and in a team environment.
Strong preference will be given to a person of Indigenous ancestry.
Job Duties:
Plan and develop the program in consultation with the supervisor
Formulate program policies and procedures and evaluate the program in consultation with the supervisor
Engage with the Coordinated Access and assist in the maintenance of the HIFIS database
Promote public awareness of and support for the program by performing duties such as producing promotional materials
Conduct housing priority needs assessments with seniors and establish service plan goals and interventions.
Assist in discharge planning in collaboration with CPAS and SHA Mtis and First Nation Health Services, SHA, and caregivers
Connect seniors to appropriate housing options, home supports, and healthcare providers, ensuring stable housing and reducing hospital readmissions
Liaise with community service providers, professionals, and landlords to coordinate service provision, facilitate referrals to the program, and represent the program in external events
Provide follow-up support to individuals and maintain ongoing relationships to ensure stable housing.
Provide landlord support, education, and conflict resolution between program client and landlord to prevent housing breakdown
