Job Summary
Under the direction of the Manager, and as part of an interdisciplinary team, the Assessor Coordinator Degree - Senior is responsible to provide a complex range of screening, assessment, treatment and case coordination services to the community mental health and addictions population seen at the Urgent Care Center. The incumbent will provide consultation and coordination of service including prioritizing and referring to community agencies. Also provides evaluation of clinical interventions, supervision and professional and public education regarding mental health issues and resources. The Assessor Coordinator Degree - Senior will provide leadership and clinical expertise in the development and maintenance of community mental health and addiction treatment programs.
The Assessor Coordinator Degree Senior, along with community and the Urgent Care Center teams, provides crisis intervention, support, assessment and follow-up plans to clients in the community who are in crisis or require assistance. The Assessor Coordinator Degree - Senior also assists in the successful stabilization and/or transition of acute patients from UCC to hospital/detox or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those who present in crisis.
