Description

The PMSS Liaison reports to the Post Majority Support Services Manager and is responsible for developing, coordinating, and overseeing outreach and establish Youth Support plans designed to empower youth ages 16 years to 17 years. The PMSS Liaison will work directly with youth who are preparing to age out of the Child Welfare System. The PMSS Liaison ensures youth are connected to resources and supports in a pre-planning stage before they reach adulthood.
Complete Outreach to Child and Family Services Providers to identify Youth who are in care; provide initial and on-going contact to promote Post Majority Support Services.
Assist the Youth to create and implement Post Majority Services Youth Support Planning and Goal Setting.
Assist and support First Nation Youth in care and their families involved in child protection case planning and legal proceedings where applicable.
One of the key responsibilities of the Family Liaison is to act as a liaison between First Nation Youth, Families, Yellow Thunderbird Lodge-YTCCFS and the Ministry of Social Services and Out of Province Child and Family Services.
This role is instrumental in facilitating and improving working relations between social workers, First Nation, the youths parents and families, enhancing overall well-being.
QUALIFICATIONS:
Certificate/Diploma in related discipline and demonstrated community work experience; or Bachelor of Social Work or Four (4) year University undergraduate degree in a related discipline and demonstrated community work experience
