Under the direction of the Home Care Coordinator, the LPN will be responsible for effectively assessing clients to ensure they receive the best care The LPN is responsible for screening, referring and assessing and treating the clients as per the regulatory Home Care services and ensure they are consistent with all polices of the Sk Home Care Program.
RESPONSIBILITIES:
Ensure nine (9) essential services of HCC program are being demonstrated
Provide case management & develop care plans based on assessed needs.
Utilize interpersonal communication and written skills in providing an objective approach to clients needs and maintain accurate client files.
Work collectively with the GGFN HCC Team and GGFN Diabetes Team.
Providing information with the Physicians and Hospital Staff on the GGFN Home and Community care clients admission to promote continuity of care and facilitate early discharge.
Plan and arrange for GGFN HCC services so the client receives the needed care and support on discharge from hospital.
Act as a resource person and professional liaison for the client and the family.
Collaborates with and maintains communication with the hospital personnel regarding referrals to the GGFN HCC to meet the client needs of GGFN.
QUALFICATIONS:
Must be a licensed LPN, in good standing with the Saskatchewan Registered Nurse Association.
A minimum of three years experience in home and community care setting.
Willing to work a variety of hours which may be evenings and weekends.
Ability to keep accurate written records.
Must have current C.P.R. certification.
Previous related experience working in a First Nations community is an asset.
Valid Saskatchewan Drivers License.
CPIC and Vulnerable sector to be completed annually.
Must have a valid Third-Party Liability Insurance if using own personal vehicle.
