Description

Intensive Case Manager shall provide support services, referrals, advocacy, and system navigation to those who are experiencing homelessness or are housing insecure.
Using a client-centered approach, provide case management services for up to 12 participants to enhance their independent living capabilities and promotion of self-efficacy; Establish and maintain effective working relationships in a multi-disciplinary team environment by providing comprehensive supports to their individual caseload; Complete intake process, assessments, develop life-skills and recovery plans, provide case management and crisis intervention with their individual caseload and prospective clients; Provide physical assistance as needed to clients in the areas of grocery shopping and apartment maintenance; Organize and participate in moving of clients as required; Facilitate referrals and access to emergency services during times of crisis, acute mental health challenges, and suicidal ideation; Arrange and participate in case conferences, doctor appointments, and other relevant meetings in conjunction with colleague agencies as needed; Work with landlords or other housing providers to enhance landlord-tenant relationships, provide advocacy, and ensure sustainability of housing; Develop and maintain trusting, healthy, therapeutic relationships with clients; Develop and facilitate support groups aimed at improving client quality of life;     Provide access to financial trustee services and financial management to those clients requiring supports; assist clients with their medication management; Maintain complete, accurate, and comprehensive case notes; Provide supervision and direction to the Housing Support Workers; Establish and maintain effective collaborative working relationships with other partner agencies; Participate in the on-call services rotation to provide crisis intervention consultation as required to other staff.
