Job Summary: Reporting directly to the Program Manager, the Nurse Practitioner is responsible for comprehensive assessments, planning, implementation, prescribing medications, and evaluation of patient care. They will perform a variety of nursing duties by administering safe, effective nursing according to clinical policies, procedures, and evidenced-based nursing care standards. The Nurse Practitioner works within a concurrent capable, recovery-oriented framework, and will oversee the RNs & LPNs and be part of the direct daily provisions.
Job Skills:
Provide primary care to patients experiencing withdrawal and to patients in need of medical stabilization.
Perform physical exams and patient observations, record patient medical histories and symptoms.
Create patient care plans and contribute to existing plans.
Order, administer and analyze diagnostic tests.
Diagnose health issues; Prescribe necessary medications as required.
Detect changes in a patients health and change treatment plans as necessary.
Administer medicines and treatments.
Evaluate responses to treatments and medications.
Orient and motivate clients toward appropriate daily functioning, life skills, physical health mental health and substance use support services from a culturally sensitive lens.
Provide supported referrals to external service providers.
Assess health, mental health, substance use and functional challenges within scope of practice.
Track and document client progress along care plans, internal databases, and case files.
Liaison with and provide on-going support to referring agencies and client support workers regarding clinical issues
Contribute to joint client service planning and case reviews.
