Key Duties and Responsibilities:
Assessment and Evaluation:
o Conduct comprehensive assessments of clients' physical, cognitive, and psychosocial functioning using standardized and non-standardized tools.
o Evaluate clients' needs related to activities of daily living (ADLs), instrumental activities of daily living (IADLs), work, leisure, and social participation.
o Identify clients' strengths, challenges, and goals.
o Document assessment findings and recommendations clearly and concisely.
Intervention Planning and Implementation:
o Develop individualized treatment plans in collaboration with clients, their families, and other healthcare professionals.
o Select and implement evidence-based interventions to address clients' needs and goals.
o Provide therapeutic activities, adaptive strategies, and assistive devices to promote independence and participation.
o Educate clients and their families about their condition and strategies for managing their challenges.
Rehabilitation and Adaptation:
o Provide rehabilitation services to clients recovering from injuries, illnesses, or disabilities.
o Assist clients in adapting their environments to maximize their independence and safety.
o Recommend home modifications, assistive technology, and other supports.
Collaboration and Communication:
o Collaborate with other healthcare professionals, educators, and community agencies to provide coordinated care.
o Communicate effectively with clients, families, and colleagues.
o Participate in team meetings and case conferences.
