Intensive In-Home Support Worker will work to provide a broad range of psychosocial and health-related supports to individuals in accordance with the programs goals and objectives. The program will provide an intensive response to family crisis to ensure the personal safety of children while allowing them to remain in the family home.
DUTIES AND RESPONSABILITIES:
Active engagement with families who are at high risk for interventions from the Ministry of Social Services. Support clients using a collaborative, person-centered approach grounded in harm reduction and trauma-informed practice.
Use brief assessment, active listening and crisis response skills to help meet immediate, basic needs and make appropriate referrals to other health and support services.
Provide outreach-based support in the community including accompaniment to appointments, home visits, eviction prevention, and advocacy in accordance with the established client care plans.
Work in collaboration with partner agencies to provide a continuum of services
Maintain both paper and electronic confidential client files as well as stats
Maintain the safety of all program spaces by following protocols related to violence prevention and critical incidents
Engage in planning, evaluation, and program development as directed.
Perform other duties as assigned

MINIMUM REQUIREMENTS:
Post-secondary education is required in health or social services programs (Social Work, Psychology, Mental Health & Addictions Support Worker, etc), or equivalent combination of education and experience. Preference will be given to individuals with two or more years of related experience in the human services field.
Valid Insurable Saskatchewan Drivers License, travel outside the city may be required
Police security check is required
