Description

We are seeking a compassionate and dedicated Child and Family Wellness Case Manager to join our team. Under the supervision of the Director of Child and Family Wellness, the Case Manager will work to protect and enhance the health, safety, and well-being of children and families within the community.

Key Responsibilities:
Provide case management services for assigned children, collaborate with family support providers, and ensure alignment with program policies.
Assess the needs of children and families, identify risks, and develop treatment plans.
Offer individual, family, and group counseling, and advocate for clients with external service providers.
Maintain confidentiality per program standards and incorporate Indigenous culture and traditions into case management.
Provide services in diverse settings, including homes, schools, and community environments.
Participate in ongoing professional development and perform additional duties as assigned by the program manager.

Qualifications:
A bachelors degree in social work is required, those with a masters degree in social work are encouraged to apply.
Previous experience working with families impacted by trauma, addiction, emotional, or mental health challenges.
Experience working within Indigenous communities and understanding the impact of generational trauma.
Knowledge of the Child and Family Services Act, Mental Health Act, and other relevant legislation.
Strong interpersonal, communication, and organizational skills.

Additional Requirements:
Must be able to work evenings and weekends as needed.
Valid drivers license and access to a reliable vehicle.
Pre-employment substance testing
Criminal records & vulnerable sector check.
