Description

Responsible for the establishing trusting and supportive relationships with assigned individuals.
Maintain caseload as assigned.  Provide intensive in-home services to Youth/Families identified by the Ministry of Social Services and EHC.  Attend Case Management meeting(s) with Ministry of Social Services and with YM Team.
Work within the Child and Family Services Act guidelines and principles.
Assist in linking and supporting youth/families to access community services and resources.
To provide practical support that enables youth/families to meet their goals: transporting to appropriate appointments, recreational services, and where necessary to reach goals and objectives of the contracts.
Be responsible to assist with any group/parenting/life skills development, planning, and facilitating.
Assist/Support/Encourage to develop skills i.e. problem solving, daily/life/coping skills, etc., which includes money management, Education Planning, job seeking skills, etc.
Prepare daily, weekly and monthly written reports in a timely manner.
Responsible for attending all staff meetings, case management meetings, team meetings, as well as any other internal/external, in-service training/professional development meetings as requested.
Adhere to the emergency/critical incident protocols and procedures.
Adhere to the confidentiality and conflict of interest guidelines and duty to report policies
Provide crisis management, act as a liaison, an advocate, and provide transportation when necessary.
Assist in the facilitation of life skills relevant groups & sessions
Must be willing to work evenings, weekends, participate in community events, fundraising activities and other such related duties as assigned.
Provide Criminal Record Check.
Provide a Drivers Abstract with a current Drivers License
CPR/First Aid Certificate.
