Job Description:
The Social Worker (Outreach) will apply the principles of health promotion, population health, and harm reduction to positively impact individuals, families, groups and communities. In addition to providing client care coordination and support, will aim to improve the social determinants of health in the region through advocacy and partnerships. The Social Worker (Outreach) collaborates within a multidisciplinary team to establish, monitor and carry out specific care plan activities to link, engage and retain clients to care using a harm reduction model.

- Valid Class 5 driver's license
