Program: Family Preservation Program
Hours: 35 hours per week, flexible field hours
Location: Ministry of Social Services Yorkton Boundary Area
Start Date: January 6, 2025
The Family Preservation Worker focuses on providing supports and services to vulnerable and at-risk families, through flexible and intensive home visitation, referrals and collaboration with other community services, to assist families to develop the skills necessary to keep children safe and to keep the family intact when it is possible to do so. This position will collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate client care and support in consultation with individuals and their families.

Expectations:
Able to communicate complex and sensitive information
Provide a high standard of care and support for clients
Schedule, prepare for and attend regular in-home visits with families
Provide resources, education, opportunities, motivation and support
Assist with and provide transportation if needed
Refer to and coordinate with other agencies as required
Complete reports in a quick, efficient and accurate manner
Adhere to confidentiality and safekeeping of clients files and records
Observe organization policy and procedure and OH&S standards
Qualifications, Experiences and Requirements:
Post-secondary education in a human service related field or an equivalent combination of work experience and education
Valid Drivers License and a safe, reliable vehicle
Minimum of two-years experience working with children and families
Experience in case management, documentation and file maintenance
Recent (6 months or less), clear Criminal Record Check and Vulnerable Sector Check
