The HCRN provides assessments for community band members needing assistance at home. The services provided are client and family focused, including initial assessment, one-on-one teaching, treatment, palliative care, and rehabilitation to prevent deterioration, and to promote independence.
The HCCN is part of the Home Care Health Team and is responsible for liaising between patients, families, and health care providers. The HCCN works to ensure patients and families understand their conditions and treatment and to ensure the clients are receiving appropriate care. The goal is to minimize the effect of the disability and/or illness, support independence and wellbeing of clients and to provide their informal support. The scope of the responsibilities of this position is to be a holistic approach to health and that health care is delivered on an individual basis or as part of a multi-disciplinary team. The HCRN is a support role and will report to the Home & Community Care Coordinator and the Director of Health.
