Reporting directly to the Home Care Supervisor, this position is part of a multidisciplinary team that provides client focused, family centered care to individual's families and communities of BRT6HC. The Chronic Conditions Nurse (CCN) is responsible to provide expertise education and guidance to the Chronic Conditions Team, work with the team to establish care strategies for chronic diseases that are prevalent in the community, plan and develop, as well as implement and direct, a program that provides client centered care for chronic disease management. The CCN coordinates a database of statistics and narratives for tracking progress and efficacy of treatments and programs outcomes.
QUALIFICATIONS
* University degree in bachelor's or Master of Nursing
* Three years of expertise in Home Care and direct Diabetes, Cardiovascular and other chronic diseases with direct education experience
* Certification as a Diabetes Educator with the Canadian Diabetes Association
* To be a registered nurse in good standing with the College of Registered Nurses of Saskatchewan (CRNS)
* CPR and First Aid
* Understand the principles of trauma informed care and how to create safe spaces for clients
* Excellent communications skills, both written and verbal
* Demonstrate leadership skills
*Ability to work independently and strong interpersonal skills
* Must possess Valid drivers license.
* Must provide a clear criminal record check with vulnerable sector.
