Description

We are looking for a Self-directed, independent, outgoing and strong interpersonal License Practical Nurse to join our Home & Community Care team. The candidate will be providing specialized nursing care for home care and personal care clients, and also providing palliative nursing care and coordinating in-home respite services.  If you feel like this would be a good fit for your personality, we encourage you to apply!
About the facility: The Home & Community Care program is based out of the Jeannie Bird Clinic. This position will provide community-based home care nursing services to community members and along with the 4 surrounding communities of LLRIB.
About the position: the Licensed Practical Nurse must be able to work under pressure, while performing nursing treatments and procedures as ordered by the physician such as catheterizations, catheter irrigation, injections, sterile dressings, wound care, monitoring blood pressures and blood glucose, etc. the LPN will also be responsible for evaluating client progress, updating the care plan based on on-going nursing assessments.
Duties:
Providing specialized nursing care for home care and personal care to clients as required;
Evaluating client progress and updating care plan on an on-going nursing assessment;
Performing rehabilitative nursing procedures in consultation with Occupational Therapist and Physical Therapist;
Providing palliative nursing care and coordinating in-home respire services;
Qualifications:
Registered and in good standing with the Saskatchewan Association of Licensed Practical Nurses (SALPN), or eligible to apply;  
Acute or home care experience is an asset;
Must be able to work independently, be self-directed and demonstrate organizational skills;
Proficient computer skills and experience using Microsoft Office;
Excellent verbal and written communication skills;
Excellent problem solving & organizational skills;
