UNDER THE DIRECTION OF THE POST MAJORITY SUPPORT SERVICES MANAGER, THE POST MAJORITY SUPPORT WORKER WILL BE RESPONSIBLE FOR EMPOWERING OUR YOUNG COMMUNITY MEMBERS WHO ARE AGING OUT OF CARE AND/OR ARE YOUNG ADULTS FORMERLY IN CARE. THE POST MAJORITY SUPPORT SERVICES WORKERS WILL DEVELOP AND IMPLEMENT EFFECTIVE STRATEGIES TO SUPPORT THE YOUTH AND YOUNG ADULTS TO REACH SELF-SUFFICIENCY WITHIN THE COMMUNITY.
Develop support programs that would aid the out-of-care young adults to easily access culturally appropriate supports and services that meet their needs.
Coordinate, schedule, and attend case conferences and appointments with community providers.
Collect and document data to track our members successes.
Assist in goal setting based on individual needs assessments.
Maintain regular contact with a network of service providers
Participate in weekly staff meetings with the management team to evaluate the program and the community members progress.
Complete appropriate paperwork for program reporting and communication.
Coordinate family reconnection of out of care young adults.
Adhere to all KSNs policies and procedures.
Maintain confidentiality and professionalism.
