Case Managers provide personalized 1:1 support and collaborate with the team to facilitate group sessions, delivering key vocational services that empower clients to succeed in the workforce. They conduct career assessments, offer guidance, and develop tailored education and employment plans. Additionally, they lead group programs, enhance employability skills, and connect clients with training, job opportunities, education, and community resources.
Primary Responsibilities:
Organizational Commitment: Collaborate with team members to achieve strategic goals and participate in community engagement.
Professional Development: Engage in ongoing training and evaluation to enhance service delivery.
Case Management: Conduct assessments to develop and monitor strength-based action plans that address individual goals and challenges.
Reporting: Ensure accurate collection and reporting of program data, participating in evaluations with leadership.
Customer Service: Foster positive interactions through empathy, adaptability, and clear communication.
Experience:
Minimum two years experience in career counselling, particularly in the human service sector and in community based organizations.
Experience working with clients from various cultural, economic and social backgrounds.
Compensation:
$48,921.91 annually
