Duties and Responsibilities:
Function as the case manager and support for participants that have been referred for program development and person-centered plan review/redesign.
Work collaboratively with program participants, their support network, Program Coordinators and staff teams to assess and interpret behaviour; provide training, coaching and support during program development.
Design, develop and implement programs and strategies to positively support program participants in their day program and personal goals; maintain ongoing support and review.
Monitor, document, provide information and interpret interaction reports for all program participants; identify support needs and complete monthly reports.
Plan, design and facilitate formal lessons or programs that address the individual needs of program participants; sessions may include (but are not limited to) Circles, Be Cool, communication skills, coping strategies, self-management skills, relaxation etc.
Develop effective working relationships with all staff and program participants to ensure a respectful and positive work environment is achieved.
Prepare documentation and reports as required by Cosmopolitan Industries Ltd.
Attend Person-Centered Planning meetings as needed.
Provide general supervision as scheduled.
Provide coverage during staff shortages.
Cross-train for other duties/positions as deemed necessary.
Other duties as assigned.
Certificate, Diploma or Degree in one of the following disciplines Disability Support Worker, Social Work, Psychology and/or a related field.
