Description

Duties and Responsibilities
Accompany and support program participants, on a 1:1 basis or in small groups, to participate in community-based programs such as supported work, volunteer work, fitness and leisure, recreation activities, community events etc.
Provide feedback for Person-Centered Plans and Comprehensive Behaviour Support Plans; monitor plans for relevance and participation and report any concerns to the Program Coordinator.
Develop effective working relationships with all staff and program participants to ensure a respectful and positive work environment.
Prepare documentation and reports as required by Cosmopolitan Industries Ltd.
Provide physical and/or personal support to participants as necessary.
Provide transportation to and from community-based programs.
Administer and record use of prescribed medication as necessary.
Ensure the maintenance and care of all necessary equipment and supplies.
Assist participants to sort out options, adhere to plans of action while providing necessary support and assistance.
Review participant background information, track information that may be used for intake reports.
Supervise program participants and provide activities in day programs.
Under the assistance of program coordinator implement life skills workshops, behaviour management programs and other community and social service programs under the supervision of social services or health care professionals.
Assist in evaluating the effectiveness of treatment programs by tracking program participants behavioural changes and responses.
Respond to crisis situations, when necessary.
Offer guidance and support that will encourage independence and assist participants in achieving their personal goals.
Implement and organize the delivery of activities and support within the community.
Other duties as assigned.
