Description

Sanctum Care Group's Prenatal Outreach Resource Team (PORT) is seeking 1 Case Manager to join our team, which is committed to delivering high quality, individualized holistic care.

The Case Manger will provide support and services through a harm reduction and trauma informed lens in a community setting. This position provides intensive case management to prenatal and postnatal women who are affected by addictions, mental health, poverty, and at risk of having their child apprehended at birth.

Case managers will provide coordination of psychosocial components of individual care plans, facilitate referrals to appropriate agencies which will assist in attaining individual goals. Provide supports and advocacy for clients with multiple issues affecting their ability to parent.

Qualifications

Ability to communicate and maintain an effective working relationship with clients from diverse social, economic, and cultural backgrounds, and maintain an effective working relationship with other members of team and health care providers.

Knowledge and awareness of Indigenous culture, history with emphasis on child welfare, and impacts of trauma.

Knowledge of service providers and supports related to this clientele

Experience working in an outreach setting

An understanding of the issues related to addiction including harm reduction

Conflict resolution and interpersonal skills

Background working in Mental Health and Addictions

BSW or combination of education and work related experience

Must have a reliable transportation and valid drivers license
