Your role
Contribute, review and implement person-centered plans designed to support and assist adults, in achieving their personal goals
Collaborate with other staff to facilitate meaningful, high quality community connections, personal development and recreation opportunities.
Encourage and support individual independence, choice, and self-expression in the areas of self care, communication, recreation, socialization and participation.
Act as a role model while encouraging and assisting the individual to participate in the daily activities and responsibilities of their personal life and their home
Support residents to participate and engage in the community through leisure activities, and fostering social networks that align with the individual's interests and person centered plan
Establish and maintain rapport with residents and maintain positive relationships with key stakeholders
