Description

We are seeking a highly motivated and compassionate individual to join our team as an Intensive Case Support Worker (ICSW). As an ICSW, you will be responsible for providing comprehensive support to the case management services being provided to individuals from vulnerable populations. This is an opportunity to make a difference in the lives of individuals in need while working with a dedicated team of professionals. Reporting directly to the Housing Programs Manager, the Intensive Case Support (ICS) Workers are responsible for supporting the Intensive Case Coordinators with the implementation of case plans for male clients that are housed in a 24-hour 7 day a week supported housing environment. This position will provide an expansion of basic needs services and expansion of intensive case management based out of the Nwo Ytina Friendship Centre that integrates with existing internal and external community resources.

Job duties:
Overnight, weekend and holiday supervision of clients housed in a 24-hour 7 day a week supported housing environment.
Provide in home life skills mentoring.
Work alongside other NYFC program supports to ensure clients have access to a wide array of supports promoting a holistic approach to over-all wellbeing.
Work in a team environment to promote centre functions and cleanliness of the building. Assist other program support staff as needed to ensure all programs are operating efficiently.  
Complete daily debriefing and entering case plan notes on interactions with clients.
Adhere to organizational policies and procedures.
Duties as assigned.
This position will require on-call work as assigned and as required to support 24/7 availability of services to clients.
