Description

Holds overall responsibility of overseeing the daily house activities/operations.
Organizes and facilitates participant care by assessing and evaluating participants' needs.
In coordination with the Executive Director and SHA, develops participant  goals and areas of need by creating care plans which are assessed regularly.
Assists participants in making knowledgeable choices, achieving wellness and autonomy by presenting them with programming and service provider options.
Coordinates and monitors services by including comprehensive tracking of participant activities in relation to the care plan.
Handles case assignments by drafting service plans and reviewing case progress.
Keeps records of cases in check by accurately completing all necessary forms and producing statistical reports.
Identifies and provides emergency crisis services as necessary and responds according to accepted crisis involvement.
Records participants' progress through monitoring referrals, appointments and other notable interactions.
Develops effective working relations by cooperating with staff,  personal and professional networks and partners throughout the entire case management process.
Adheres to professional standards as outlined by protocols, rules, and regulations.
Work with staff to develop capacity through conversations, debriefs, direct question and answer sessions, sharing of knowledge and resources, etc.;
Communicate with workers around ongoing issues with participants and put initiatives in place to address issues. Debrief with staff following incidents and disclosures;
Promote procedures and processes that ensure quality services and the safety of the youth, facility and staff;
In Consultation with the Executive Director, delegating tasks to appropriate staff as and when required;
