Description

In this role you will be responsible for assisting individuals with disabilities in developing and maintaining the skills necessary for independent living. This role involves providing direct support, resources, and advocacy to help clients achieve greater autonomy and integrate fully into their communities.

Key Responsibilities:

Assessment and Planning:
- Conduct initial and ongoing assessments of clients' needs and abilities.
- Develop individualized service plans in collaboration with clients and other stakeholders.
- Set realistic, achievable goals for independent living skills development.
Skill Development:
- Teach and support clients in daily living skills, including personal care, household management, budgeting, and nutrition.
Resource Coordination:
- Connect clients with community resources and programs
Advocacy and Support:
- Advocate on behalf of clients to ensure their rights and needs are met.
- Mediate conflicts and help clients resolve issues related to independent living.
Documentation and Reporting:
- Maintain accurate and up-to-date records of client interactions, progress, and services provided.
Community Outreach:
- Develop and maintain relationships with local organizations, service providers, and community groups.

Qualifications:

Experience working with individuals with disabilities or in a related human services field is preferred.
Experience in teaching or coaching life skills is highly desirable.

Skills:
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Excellent organizational and time management abilities.

Other Requirements:
- Valid drivers license and reliable transportation.
- Clear criminal record check
- Flexibility to work evenings or weekends as needed.
