Job Description: Reporting to the manager of Community Mental Health and Addiction Services, the Community Mental Health Nurse works collaboratively with a multidisciplinary team, as an integral member, providing intensive case management to individuals with chronic and persistent mental illness. These services are provided within an individual and group context and working closely with psychiatry, supporting client needs. Responsibilities include intensive case management, medication management, crisis intervention, coordinating referrals, counselling, goal planning, consultation to community agencies, group/community education, student mentoring, documentation, data collection, client/family and public education, advocacy, and mental health promotion.
Required Qualifications
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing
Current CPR and PART certification
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS) or Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
Valid driver's license and access to a reliable vehicle
Experience
Five years recent direct clinical experience in providing community-based case management services to clients with severe and persistent mental illness.
Knowledge, Skills and Abilities:
The Community Mental Health nurse must demonstrate an understanding of the
principles of:
-Nursing process and interventions in relation to mental health of the individual, including the ability to utilize a variety of assessment tools, i.e. Mental Status Exam, BPRS, and Suicide Risk Assessment etc.
-Mental Health Services Act and related legislation
-Mental Health Promotion and advocacy in mental health serv
