Job Description: Under the direction of the Manger of Rehabilitation and Community Services, and as part of an interdisciplinary team, the Social Worker BSW provides
a range of screening, triage, assessment, treatment and case coordination services to primarily adult community mental health and addiction
population.
Utilizes a team approach to provide screening and/or assessments; prioritizing referrals; provides suicide risk assessments; crisis intervention; family
support services; case management services; clinical assessment and effective interventions to persons suffering from mental health and addictions
issues.
Provides education and support to others in the community regarding mental health and addiction client needs and mental health and addiction issues.
Application of the Mental Health Services Act and Health Information Protection Act.
Provides coverage to client's receiving services.
Maintains client records as per SHA documentation standards.
Participates in clinical audits and attends meetings as required.
Consults with program area manager, Sr. Clinicians, Discipline Heads, and other colleagues as appropriate regarding difficult clinical situations, legal
and ethical issues.
Works collaboratively with both internal and external service providers of the SHA in the provision of services to individuals with mental health and
addictions issues.
Liaise and advocate with various community agencies to ensure that clients with mental health and addiction issues can access and utilize community
services appropriately.
Assessment of community resources, identifying needs and gaps in services.
Participates in assigned community development projects as required.
Responsible for conducting self in a safe and responsible manner as outlined in the Occupational Health and Safety Act.
