Description

Under the supervision of the Director of Social Programs, or her designate, the Community Outreach Caseworker will assist in case management and in the field service delivery of the Community Outreach Program, as they provide direct services for hard to serve youth in care of the Ministry.  

In the field work with the youth in the program, including:
Make the necessary services as easily accessible as possible for youth in the program
Promote positive independent problem solving and decision making skills
Provide information and life skills support to youth related to personal, family, housing, healing, safety, work, school, income, etc.

Assist in case management and support for youth in the program, including:
Intensive case management
Provide stabilization crisis support
Provide incidental problem solving and emotional support to youth
Provide emergency, incidental behavior management (non-violent crisis intervention)
Refer youth to support services with other agencies (ex: income support, employment, counseling, addiction services, mental health service, etc.)

In addition:
Role model positive behavior and encourage, and assist youth to practice self-help and non-violent problem solving skills daily
Ability to provide effective and accurate written documentation (ex: case management notes, incident reports, referrals to outside agencies, etc.)
Maintain confidentiality
Perform other related duties as assigned/needed
