The Family Preservation Worker, directed by the Program Manager, provides supports and services to vulnerable and at-risk families through flexible and intensive home visitation, referrals and collaboration with other community services, to assist the family to develop the skills necessary to keep children safe and to keep the family intact. This position will collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate client care and support in consultation with individuals and their families.

Schedule, prepare for and attend regular in-home visits with families that meets the familys needs
Provide relevant resources, education, opportunities, motivation and support to families
Assist with/ provide transportation if needed
Refer to and coordinate with other agencies as required
Complete reporting standards in a quick, efficient and accurate manner
Adhere to confidentiality and safekeeping of clients files and records
Observe program and organization policy and procedure and OH&S standards
Qualifications/Experience:
Post-secondary education in a human service related field or an equivalent combination of work experience and education
Valid Drivers License and a safe, reliable vehicle
Current and clean Criminal Records/Vulnerable Sector Check
Minimum of two-years experience working with children and families
Experience in case management, documentation and file maintenance
Recent clear Criminal Record Check and Vulnerable Sector Check
