Job Description: Under the direction of the Executive Director of Community Services, within the context of a client and family centered care model and in accordance with the vision and values of Athabasca Health Authority, this position is responsible for providing enhanced service coordination to children and families in the five Athabasca Health Authority communities. The Jordans Principle Service Coordinator will work with families: to help them at many points along the health care continuum. This may include referrals to specialists for their children, understanding treatment and care options, understanding insurance benefits, serving as coach and quarterback of their health care team, working with family members and caregivers, mobilizing resources, managing medical paperwork and finding shelter/housing options. Jordans Principle Service Coordinator will support the restoration, maintenance and enhancement of social functioning by mobilizing strengths and supporting coping capacities of families.
The Service Coordination will have three main activity areas:
Case Management
Provide a collaborative, client driven system to guide children and their families through a comprehensive array of services
Support a child and family centered approach to coordinating and connecting services
Work towards establishing multi-disciplinary hubs for families when the services they require involves a number of professionals
Intake, Assessment and Coordination
Help families navigate the health and social systems in a timely manner
Help with referrals for assessments to appropriate professional, or provide assessments within their scope of practice
