Job Description: As a member of Mental Health and Addiction Services, the Community Connecting to Care Nurse is an integral part of a multi-disciplinary team providing intensive case management to individuals with chronic and persistent mental illness and substance abuse issues and other chronic diseases. These services are provided within an individual and group context. Responsibilities include intensive case-management, medication management, crisis intervention, counseling, goal planning and consultation to community agencies, group/community education, student mentoring, documentation, client, family and public education as well as mental health promotion.
Required Qualifications
Registered or eligible for registration with the Registered Psychiatric Nurses Association of Saskatchewan (RPNAS) or Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 driver's license
Bachelor Degree in Nursing or Bachelor Degree in Psychiatric Nursing
Current CPR , PART and WAVE certification
Experience
Five years recent direct clinical experience in providing community-based case-management services to clients with severe and persistent mental illness. Recent psychiatric nursing experience
MENTAL SKILL:
Requires a high level of judgement in determining relevant areas to focus on in assessing an individual, e.g. safety issues, living situation, compliance with medications etc. Requires resourcefulness and ability to gather and sort relevant information. Requires initiative and professional judgement in planning appropriate interventions with the multidisciplinary team. Requires team building skills, leadership, assessment, communication and goal planning skills.
