Description

Home & Community Care Nurse position based out of Spiritwood & working in 1st Nations communities of Big River, Pelican Lake & Witchekan Lake. Responsibilities include client assessments,client monitoring, home visits, referrals, delivering Wellness/Footcare Clinics. Responsible for liaising between clients, family, community & health care providers. The HCCRN is part of the Homecare Team & reports to the HCC Nursing Supervisor
