Job Description: Under the direction of the Manager of Rehab and Residential Services the Assessor coordinator ( MHT) will be responsible to assist in the successful transition of acute psychiatric patients from hospital into community or alternatively to prevent admission /readmission by providing short term acute mental health in home/ community services. Service provision includes: screening/ assessment, developing care plans, coordination with other service providers and direct mental health homecare support.
Required Qualifications

Registered or eligible for registration with the appropriate licensing body
Recognized degree/diploma in nursing or social work degree
Knowledge, Skills and Abilities

Basic computer skills
Problem solving skills
Decision making skills
Knowledge of community resources and agencies within the Prince Albert Parkland Health Region, surrounding communities and First Nations reserves
Excellent verbal and written communication skills
  Demonstrated above average clinical assessment skills, case planning, consultation and supervision  
Demonstrated leadership skills, workload management and administration skills
Good public relation skills and ability to establish and maintain effective work relationships
Knowledge of a variety of mental health disorders and interventions required to sustain stability
Able to work independently and in a self-motivated manner, ability to respond under pressure
Knowledge and practical application of the Mental Health Services Act
