Job Description: Reporting to the Director,  Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care services to assigned populations which may include: urban populations, visiting services to  rural communities, and/or priority populations such as newcomers, as determined by community and program needs.   Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with and referral to other health care providers; participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation. Depending on community requirements, the position may include first on-call support.
Required Qualifications

Educated in a Nurse Practitioner program in a Canadian province/territory
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA) as an RN (NP) or RN (GNP)
Experience
    Minimum of three (3) years experience working as a Registered Nurse, with experience as RN (NP) in a Primary Health Care setting preferred;

This is a multi-site position.

224.00 hours per 6 week rotation: 28 shifts of 8.00 hours
