Duties and responsibilities:
- Collaborate with HUB Coordinator, Supervisor, and other Support Workers to facilitate meaningful, high-quality individual and group programs and activities.
- Promote independence, self-esteem, and inclusion in the areas of self-care, communication, recreation, socialization and participation.
- Provide supportive assistance to ensure the physical, emotional and social well-being of participants.
- Provide supervision of participants while participating in community and/or in-centre activities.
- Transport participants to community activities.
- Promote, instruct and model acceptable social behaviour.
- Maintain participant documentation according to agency standards including completion of Incident Reports, contact records and file recordings, and contributing to and reviewing person-centred plans to ensure goals are being met.
- Maintain participant records including attendance, goals and communication books with families and caregivers.
- Administer all areas of personal care, as required.
- Maintain cleanliness of program areas including kitchen space and washroom.
Qualifications:
- Disability Support Worker Certificate (Rehabilitation Worker Certificate) or Life Skills Coach and/or related training and experience working with individuals of varying abilities and behaviours.
- Valid Class 5 Drivers Licence.
- First Aid and CPR certification.
- Effective time management, organization and problem-solving skills.
- Demonstrated ability to be innovative, open-minded, and creative.
- Knowledge of lifting techniques and transfers.
The final candidates are required to provide a satisfactory Criminal Record Check, including Vulnerable Sector Search.
Please note: These support positions are flexible and adaptable to the needs of the individuals that we serve and will include day, evening and weekend shifts.
