Duties and Responsibilities:
- Collaborate with Quality of Life Coordinator and other Support Workers to facilitate meaningful, high-quality individual and group programs and activities on a daily basis.
-Assess individuals in areas such as physical limitations, orientation and mobility skills, and cognitive, social and emotional barriers to establish client rehabilitation or adaptation goals
-Instruct individuals with disabilities and their families in the use of rehabilitative techniques, prosthetic devices, wheelchair's and other equipment designed to maximize clients' independence and potential.
- Provide supportive assistance to ensure the physical, emotional and social well-being of participants.
- Provide supervision of participants while participating in community and/or in-centre activities.
- Maintain participant documentation according to agency standards including completion of Incident Reports, contact records and file recordings.
- Maintain participant records including attendance, goals and communication books with families and care givers.
- Administer personal care, as required.
Qualifications:
- Supervisory and any related training and experience in hospital and care home setting and also working with individuals of varying abilities and behaviours.
-Basic Life Support (BLS) Certificate or CPR Cetification
- Effective time management, organization and problem-solving skills.
- Demonstrated ability to be innovative, open-minded, and creative.
- Effective written and verbal communication skills to express ideas and information in a clear and concise manner.
- Knowledge of lifting techniques and transfers.
