Job Description: The PHC Pharmacist will be a team member working closely with physicians, NPs, educators, dietitians and other team member. He/she will be committed to the integration of knowledge and skills in the provision of quality client-focused pharmacy services. This will include conducting medication histories, identifying problems, developing and monitoring care plans and then communicating the plan to the patient and interdisciplinary team. Consultation will be based on referrals from interdisciplinary team members. Presentations will be required to targeted patients and physicians/NPs and other team members. Will be required to participate in staff meetings, group conferences, relevant therapeutic discussions and case presentations and PHC team meetings as required.The incumbent must be able to perform in a capacity consistent with CSHP standards.


Required Qualifications

Bachelor Degree in Pharmacy
Accredited Hospital Pharmacy Residency Program
Registered or eligible for registration with the Saskatchewan College of Pharmacists
Experience
Membership in a National Professional Association is an asset

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement
