Job Description: The Patient Care Coordinator follows assigned clients from admission to discharge providing case management through assessment, coordination, implementation, monitoring and evaluation of health related services to clients. Quality care and cost effective outcomes are promoted with the client's expressed health goals and general well-being at the forefront of the care plan. Required Qualifications

Valid Class 5 drivers license
Registered or eligible for registration with the appropriate licensing body
Bachelor Degree in Social Work or Nursing
Knowledge, Skills and Abilities

Knowledge of community resources and agencies within the Prince Albert Parkland Health Region, surrounding communities and First Nations reserves
Demonstrated ability for integrated case management
Demonstrated strong written and verbal communication skills
Able to relate effectively and establish positive interpersonal relationships /skills with professional and para-professional staff within a hospital setting
Demonstrated strong problem solving and confliction resolution skills
Flexible, self-directed and capable of autonomous organization and prioritization of work
A demonstrated ability to meet and work with clients, utilizing sound judgment, tact, initiative and high standards of integrity and responsibility
