The Community case worker will provide client driven health and social case management assistance to clients with chronic conditions such as HIV, who have difficulty engaging in care.
Duties:
- Conducts case work interviews with clients, family members, service providers, and others to obtain information determining client service needs and eligibility;
- Identifies psychosocial, economic, and physical needs of clients;
- Assesses clients' support systems, available community resources, and other factors;
- Develops service plans including short term and long term goals depending on client assessment;
- Provides health information, skill development, counseling, advocacy, crisis intervention and referral through telephone and one on one contact in a variety of settings such as: clinical, home, community, correctional, and outreach settings;
- Assists clients in making arrangements for transportation to appointments as required;
- Provides workshops and continuing education relating to the social needs of clients with HIV and other related chronic / communicable diseases;
Qualifications:
- Diploma in Social Work
- Experience working with clients with multiple health issues including HIV, and mental health and addictions issues, or an equivalent combination of education, & training is an asset
- Experience in planning, scheduling and implementing work programs
- Strong analytical, organizational, and written and verbal communication skills.
- Current Criminal Record Clearance, Child Abuse Record Check and updated Driver's Abstract;
- Must have a valid driver's license; Ability to travel to outlying communities;
- Ability to speak Cree is an asset
