Community Support Worker - Outreach - Pilot Project
Reporting to the Executive Director, the Director, and in consultation with the Case Coordinator, MHAS, FHHR the Community Support Worker is responsible for engaging with clients of mental health and addictions services in the development and ractice of skills for self reliance. The Community Support Worker participates with client activities that promote client progress through clinical pathways (designed by the client and case manager. Relapse prevention plans are inclulded in the pathways. This is an integrated service directed toward youth (over 16 yrs of age) at risk and young adults.
Duties:
Typical duties and responsibilities are intended to assist clients in appropriate use of community resources and offer community supports.  The team shall provide residential supports consistent with Housing First principles for up to fifteen active clients.  
Duties include:
Provide orientation to the service to clients and work with client recovery plans
Collect and maintain statistical data including demographics and outcomes for the project and data entries to ADG information system.  This includes performance indicators.  
Provide assistance in maintenance of client housing, income support, life skills education and recovery journey as determined by the case manager  
Arrange and participate in case conferences, medical appointments and other relevant meetings with collateral agencies and significant others  
Provide crisis intervention as may be required.  Follow crisis intervention protocol as detailed in client case plans  
Assure appropriate records are maintained such as contact notes, progress reports and incident reports.  Organize and conduct group sessions in areas such as social and recreational activity, interpersonal relationships and recovery education  
Provide after hours on call, sick time and holiday coverage as may be assigned
Participate in staff meetings and training
