Description

The Lac La Ronge Indian Band Health Services is now inviting applications for a Temporary full-time Community Case Worker position for the community of La Ronge with a strong possibility of extension.  Under the direction of the Community Health Nursing Manager, the Community case worker will provide client driven health and social case management assistance to clients with chronic conditions such as HIV, who have difficulty engaging in care.
To enhance client health and wellness and improve their quality of life.

  Duties:
-    Conducts case work interviews with clients, family members, service providers, and others to obtain information determining client service needs and eligibility;
-    Work with the community partners and community members;
-    Identifies psychosocial, economic, and physical needs of clients;
-    Assesses clients, support systems, available community resources, and other factors;
-    Develops service plans including short term and long term goals depending on client assessment;
-    Provides health information, skill development, counseling, advocacy, crisis intervention and referral through telephone and one on one contact in a variety of settings such as: clinical, home, community, correctional, and outreach settings;
-    Assists clients in making arrangements for transportation to appointments as required;

Qualifications:
-    Diploma in Social Work
-    Experience working with clients with multiple health issues including HIV, and mental health and addictions issues, or an equivalent combination of education, & training is an asset
-    Experience in planning, scheduling and implementing work programs
-    Strong analytical, organizational, and written and verbal communication skills.
-    Current Criminal Record Clearance, Child Abuse Record Check and updated Driver's Abstract;
-    Must have a valid driver's license; Ability to travel to outlying communities;
-    Ability to speak Cree is an asset
