The Lac La Ronge Indian Band, Health Services Department is now inviting applications for a Temporary full-time Community Case Worker positions for the community of La Ronge with a strong possibility of extension. Under the direction of the Community Health Nursing Manager, the Community case worker will provides client driven health and social case management assistance to clients with chronic conditions such as HIV, who have difficulty engaging in care. To enhance clients health and wellness and improves their quality of life.
Duties:
- Conducts case work interviews with clients, family members, service providers, and others to obtain information for determining client service needs and eligibility
- Work with the community partners and community members
- Identifies psychosocial, economic, and physical needs of clients
- Assesses clients' support systems, available community resources, and other factors
- Develops service plans including short term and long term goals depending on client assessment
- Provides health information, skill development, counseling, advocacy, crisis intervention and referral through telephone and one on one contact in a variety of settings such as: clinical, home, community, correctional, and outreach settings
- Assists clients in making arrangements for transportation to appointments as required
- Provides workshops and continuing education relating to the social needs of clients with HIV and other related chronic/communicable diseases
Qualifications:
- Diploma in Social Work
- Experience working with clients with multiple health issues including HIV, and mental health and addictions issues, or an equivalent combination of education, & training is an asset
- Strong analytical, organizational, and written and verbal communication skills.
- Current Criminal Record Clearance & Child Abuse Record Check
- Must have a valid driver's license; Ability to travel to outlying communities; Ability to speak Cree is an asset
