Description

Main Activities
Provide supportive care to people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psycho social factors
Provide supportive care through a patient centered model aimed at creating independence and autonomy while fostering an environment of acceptance and understanding
Provide therapeutic counselling/assistance to clients who may present with multiple issues affecting their ability to cope and manage in the community
Advocate on behalf of clients for additional service and resources, as required
Personal care to patients including, assisting with grooming, bathing, personal hygiene, toileting, wound care, nutrition and all other activities associated with daily living
Assist with the transition plans from Sanctum into the community set by the client and care team
Follow medical instructions carefully and consistently
Assist with meal preparation and planning, as well as other house hold duties such as cleaning and maintenance
Manage any unanticipated events or unstable situations
Qualifications
The successful candidates will have the following skills and experience
Cultural sensitivity of First Nations issues
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Willingness to adapt to new concepts, techniques and best practices in the academic and clinical fields of chronic disease management, addictions and harm reduction
An understanding of the issues related to HIV including harm reduction
Conflict resolution and interpersonal skills
Background working in a health care setting and/ or working in Mental Health and Addictions
Degree/diploma or certificate in social work, care aid, addictions or related field
CPR/First aid/Food safety certificate/Knowledge of TLR
