Clinical support to the Diabetic & Family Outreach Project by:providing information on current best practices,program and clinical updates and new resources;providing consultation and follow up on case management issues;provide program and other statistical reports as required in order to facilitate program planning.Nutrition and diabetic education services to the diabetic clients; Educational services to the community; Follow up on referrals made be the health staff etc.
